PS 130/ MS 839 After School Program is only available to 2nd-5th graders attending MS.839 (713 Caton Ave) and P.S 130 (70 Ocean parkway).
After school Fall semester starts September 12th.
There is minimum 2 days per child requirement to register.
Important! After placing online registartion please send in Parent Agreement Form and Medical Form (click down below to access forms). A confirmation email and payment instructions will be sent to you once we receive Parent Agreement Form. Forms can be mailed/faxed to 347-497-4644 or emailed to campolympiany@hotmail.com
By placing your registration you agree that
You are a custodial parent or legal guardian of the child you are enrolling and that the child you are enrolling is healthy and fully able to participate in all camp activities. You agree to provide Camp Olympia with properly completed Parent Agreement and medical form, based on an exam performed less than one year from the date of registration, as required by the City of New York and the Board of Health prior to child attending after school program.
You hereby grant permission to Camp Olympia to take your child on all scheduled trips and outing and to photograph or record on video tape all program activities and to use any photograph or video in which your child appears in for promotional literature, displays or any other format representing Camp Olympia to the community including the Camp Olympia website. You release all rights to such.
You do hereby give permission to Camp Olympia to obtain the necessary emergency medical treatment for your child, with the understanding that the family is notified as soon as possible. If your child receives an injury during after school, you are responsible for any medical expenses incurred.
